Our staff is knowledgeable about mental health disorders and about the various treatment options and corresponding levels of care. They will listen to your concerns and ask you about your medical history and psychological history. The information you provide will allow them to determine how to best meet your needs and discuss your options with you.

Our admissions counselors work closely with Dominion Hospital physicians and can facilitate a smooth and timely hospital admission. When you choose to seek help at Dominion Hospital, our dedicated interdisciplinary team will welcome you and work with you from the day you walk in, to fulfill your goals and meet your needs, in preparation for the day you walk out.

We want your patient admission and registration process to go as smoothly as possible so we ask you to bring the following information with you:

  • Current Medications.
  • If you have an Advanced Directive such as a living will or durable power of attorney for Healthcare decisions.
  • Picture ID
  • All active insurance cards

Before coming to Dominion Hospital, patients have the opportunity to view and complete our essential registration forms in PDF. If you like, please download the following files for your information and registration purposes.

At registration you well be asked to sign two important forms, the Consent for Treatment, and Authorization to Bill Insurance. Dominion Hospital accepts most insurance plans; in addition, the admissions office will try to assign an in-network physician when one is available, although this cannot be guaranteed. We want you to know that most insurers require that hospitals and other healthcare providers keep them informed about patient progress and the prescribed treatment plan in order for the benefits to continue. Patients benefit from this system because their care is constantly being reviewed and our staff is efficient in providing the necessary communication.

Dominion Hospital is committed to keeping your information confidential. We have very strict policies to prevent anyone from obtaining your information from us without your written permission.
